Client intake, consent & waivers

Collect a health form and signed consent before the appointment — encrypted, sent automatically after the deposit, and auto-deleted on your schedule.

Linebook can collect a short health form and a signed consent/waiver from each client before their appointment — so your artist has what they need and your paperwork is done before the client walks in.

How it works

1As soon as a client's deposit clears, Linebook emails them a link to complete their forms — a quick health screening and your consent/waiver, with optional placement photos.
2They fill it in on their phone in about two minutes and sign. You can re-send the link any time, or send it from the booking's Manage screen — including to a walk-in, by adding their email on the spot (see Managing bookings).
3The completed intake is attached to the booking for the artist to review before the session.

Make the form your own

Settings → Data & privacy → Intake & consent form lets you shape both parts to your studio:

Health screening: add, remove and reorder questions. Each can be Yes/No, short text, a paragraph or a date; mark any as required; and set a Yes answer to ask the client to describe.
Extra consents: add your own consent checkboxes alongside the core three (health, waiver, photos) — an aftercare acknowledgement, numbing-cream consent, or a house rule. Mark any as required to gate submission.
Minimum age: 18 by default (standard across the US and Canada). Adjust it only if your jurisdiction differs — it fills the age line in the waiver and the client's age confirmation.
Consent/waiver: edit the title and body. Use the {studio}, {artist}, {governing} and {minAge} placeholders and Linebook fills in the right details on each booking.
There's a live preview as you edit, and a reset-to-defaults if you want to start over. Leave it untouched and clients get safe, sensible defaults.
NoteYour waiver is a legal document. The defaults are a solid starting point, but have your wording reviewed for the rules where you practise.

Kept secure

Everything a client submits — health answers, signature and photos — is encrypted, and only your studio can read it.
You choose how long intake is kept (Settings → Data & privacy; default 7 years, aligned to liability windows). Older records delete themselves automatically.
You can permanently delete any client's record on request — a clean right-to-erasure.

NoteIf the forms link ever isn't available for your studio, Linebook simply doesn't send it — clients never get a dead link.
